To get a work permit, you need to go down to the sheriff’s office. That’s what my school counselor told me. I’m in Las Vegas too, same age as you as well. I plan on getting a job at Star Bucks too! :] Umm, as for what you should put on the application, you should ask your parents for help.

Ask your parents where to get a permit in your area.
Put yes for leagally able to work in the us.
Proof is your social security card, and a drivers lisense, since your not old enough for a drivers lisense, a permit or your work permit will work. If you get an interview just ask them they will tell you!
For position simply put "any available"
salary desired I would put your minumum wage amount in your area.
For years completed put 0 since you had just started.
Subjects studied Iv always put general since it is just High school, but feel free to put those business and marketing classes down!
For former employers just put "N/A" meaning not applicable since this will be your first job.
References you can put the people you babysitted for or teachers.
